Getting started is easy

  1. Create an account
  2. Complete a simple questionnaire
  3. Fill in the data boxes
  4. Upload the death certificate
  5. Digitally sign and submit

You will receive a confirmation for all digital submissions. Government forms requiring a “wet ink” signature will be mailed to you within two weeks for review and signature. 

Once the mailed package is received:

  1. Sign where indicated
  2. Place the contents into the pre-addressed, postage-paid envelopes along with the supporting documents
  3. Drop the envelopes in the mail
